Website Release Form

Eastminster Presbyterian Church is seeking approval to have photos of activities included on the church’s website (www.eastminster-york.com).  As you may be aware, there may be potential risks associated with the posting of personally identifiable information on a web site.  For that reason we will be following these guidelines for using images of individuals in the public access portion of the website:

• Other than staff, no names will be used to specifically identify anyone in a photo.
• No personal phone numbers addresses, or other identifying information will be published.
• Should an individual or parent/guardian have an objection to a posted photo of themselves or their child the website administrator/webmaster will immediately remove it upon request.
• A release form must be on file for anyone’s image to be posted on the website.

I hereby grant the Eastminster Presbyterian Church permission to use my likeness in publications, including the website.  I also authorize the Eastminster Presbyterian Church to use any such photo for purposes of publicizing Eastminster Presbyterian Church programs or for any other lawful purpose.
